Care Coordinator


Care Coordinator required for our client who is an award-winning provider of home care services to the elderly and people with disability.

The main purpose of role is to manage the coordinating of the staff and delivery of care to service users and efficiently manage your team as instructed by the care manager(s) and partners of the company.

The position of our clients care coordinator’s regular duties can be quite varied and main duties will include the following:

  • Developing care plans and updating them as necessary.
  • Visiting service users, monitoring the care they've received and documenting anything accordingly.
  • Working with care teams to evaluate individual performances.
  • Attending workshops and liaising with professional bodies such as councils that the company works in partnership.
  • You will be required to work in the office and out in the field if required, although the role primarily office based.
  • To liaise with the senior management, ensuring that care services and on call is sufficiently covered.
  • General management of office duties, such as filing and ensuring the company is complaint with legislation/GDPR.
  • The Care–coordinator must manage issues within their team such as sickness/absence.
  • Respond to calls for new service users from councils and private clients.
  • Follow up interests and enquiries about our services by visiting service user’s homes, carrying out the required assessments (including a risk assessment).
  • Write up care plan and choose the appropriate staff to carry out the care.
  • Deal with any medical or emergency situations that may arise (this can include evenings and weekends).
  • Attend meetings or reviews with service users, social workers and families.
  • Attend office management meetings, providing useful insight into operations and future growth.

  • Full Time Permanent Role.
  • You will be predominantly in West.

    Lothian (Bathgate/Livingston) ,however you may be asked to work further a field as required.


  • The successful candidate MUST have extensive experience in care.
  •  SVQ 3  is desirable and should be registered with the SSSC.
  • Knowledge of how to recognise abuse and safeguarding procedures.
  • Knowledge of health and safety matters in relation to homecare services and risk management.
  • Commitment to respecting the rights of service users at all times and to promoting their privacy, dignity and independence throughout their time using our service.
  • Good understanding of the regulatory responsibilities of a Registered Manager and the law relating to domiciliary care services.
  • Excellent understanding of the principles of high-quality person-centred care and support and non-discriminatory care practice.
  • Full understanding of medication administration.
  • Must have a full driver’s licence with access to own vehicle.
  • Excellent timekeeper and reliable for business needs.
  • Professional manner and smart appearance.
  • Live in or around the West Lothian area.


  • £11.30 Per hour @ 37.5 hours per week
  • 35p per mile travel costs
  • 28 days holiday (pro rata)
  • Paid training which includes medication, dementia, moving and handling and Parkinson's
  • Paid mileage (allowance) and travel time
