Qualified Social Worker needed for Bracknell Forest Council, £28 ltd an hour

Start Date- ASAP
Contract End date- 3 months initially
Hours per week - 37 hours
Pay rate - £28 ltd an hour

Description
, To work as a Social Worker as part of a multi-disciplinary team.
, To provide support to older people with diagnosed memory problems who need social care because of their mental health and/or memory problems
, To work in collaboration with colleagues from health and social care to respond to urgent requests for assessment/monitoring/support to people with a diagnosis of dementia or significant mental health disorder who are presenting with a physical health problem to prevent hospital admission and to facilitate a co-ordinated discharge back into community

What is required?
, Experience of hospital discharges and the discharge to access framework
, Working experience in an Older People Mental Health setting
, Social Work Qualification
, Social Work England Registration
, Enhanced DBS

Working as a Qualified Social Worker with Service Care Solutions

, We will pay you £150 after 4 weeks in an assignment if you have no previously worked with us
, We offer £250 for every Qualified Social Worker you refer over to us
(£150 after 6 weeks in the assignment and £100 after completing 12 weeks)

, We will pay up to £250 each year to help with your Continuing Professional Development
, We will pay a “thank you” bonus to you after each 12-month period you complete with Service Care Solutions
