Winter pressure - Social Worker - Reviews Project

This is an exciting opportunity for a Social Worker to work within a reviews project team.

This is a locum Social Work position based in Berkshire.

This role is offering flexibility with working from home, the social worker will be reviewing the back log of cases caused from COVID - 19.

The social worker will be liaising with the Hospitals, carrying out CHC checklist as well as implementing care plans and packages.

Your responsibilities as a Social Worker

• Undertake Mental Capacity Act assessments


• Implement care plans


• Undertake Care Act Assessments and Safeguarding assessments


• Carry out a care package


• Attend meetings


• Carry out CHC checklist


• Use DST (decision support tool)


• Liaise with other professional within the team for the best interest of the client


• Work with Hospital to reduce the admission of the COVID backlog

Requirements of a Social Worker

• Qualified Social Worker BA/DipSW/CQSW or equivalent


• Current registration with Social Work England


• 2 years post qualifying UK experience
