Role responsibilities –Health & Wellbeing Practitioner:

Delivering client centred treatment, this Health & Wellbeing Practitioner role involves managing a caseload, providing appropriate harm reduction advice and guidance, and completing individual client assessments and care plans.

Other duties include:

  • Carrying out substance misuse and low level mental health interventions relevant to individual client needs, including 1:1 sessions and group work/programme facilitation
  • Providing transitional support from prison to the community via integrated working with other Health and Wellbeing agencies both within the prison and in the community - this is essential to prepare service users for release
  • Developing and contributing to the Substance Misuse and general Health and Wellbeing care needs of service users, supporting them in the development of release plans appropriate to their needs
  • Ensuring substance misuse and mental health groups, and Health and Wellbeing programmes, are facilitated to Forward’s standards



What we are looking for?

  • A Level 3 qualification in Counselling
  • Experience of providing counselling / CBT services to prisoners, or those with Health and Welling, or drug and / or alcohol problems
  • Knowledge of the issues facing substance misusers and low-level mental health issues
  • Demonstrable experience of carrying out risk assessments, comprehensive assessments and the design and implementation of SMART care plans
  • Significant experience in carrying out psychosocial interventions including using motivational interviewing techniques in both 1:1 and therapeutic group settings
  • A thorough knowledge of and commitment to the 12-step process of recovery from addiction
  • Ability to positively stimulate groups and clients, relating in an empathetic manner
  • Strong verbal skills and interpersonal group work facilitation skills
