Contribute to the provision of a comprehensive and specialist psychology service to the patients

Provide specialist psychological interventions through initiating and implementing treatment programmes in collaboration with other team members
Liaise with referrers, external agencies and organisations as appropriate
Produce and disseminate reports and clinical summaries of an excellent standard
Participate (under the direction of the Regional Psychologist) in contributing to regional audits as well as the research programme for the sites

She is a 13-year-old female who has tracheostomy and ventilation requirements.
This role will involve the carers overseeing health related support requirements throughout the day and night.
We are looking for healthcare workers who can deliver person-centred care in line with a personalized care plan, created specifically to meet our client's needs.
You will be fully supported by our highly skilled Nurse Managers, who are on hand to support and guide all of staff, to ensure the highest standards of care and an excellent pathway of professional development.

About the team
This community-based team works to support adults aged 18+ with learning disabilities and their carers to be able to access all necessary resources.
Completing Care act assessments, creating support plans and reviewing care packages are all key responsibilities in this role.
The team is driven to improve community inclusion for every service user to prevent isolation and does participate in duty work.
About you
Experience within an LD specific team, community work or mental health setting lends well to the type of work this team deals with.
A degree within Social Work (Degree/DipSW/CQSW) with a minimum of two years' experience working in an Adult's Social Work setting are essential for this position.
Understanding of CoP11 and relevant documents for CoP referrals is key to be successful in this team.
