It is REQUIRED to have a Social Work Qualification with at least 2 years of post qualified experience
About the Team
The team provides crucial adult social care interventions, including Care Act Assessments, MCA, Safeguarding advice, information and referrals with a focus on achieving both individual patient goals and service targets for efficient patient flow and hospital admission avoidance.
Additionally, the team supports the NETT team to facilitate a smooth transition for complex neurological patients from hospital/rehab units back into the community.
About you
Experience within a Hospital and Mental Health Team will work well for this role.
A degree within Social Work (Degree/ DipSW/CQSW) with a minimum of 2 years post qualified experience is essential.
A valid UK driving licence and vehicle are preferred but not essential.
