About the team
As a Senior Health Liaison Social Worker in this team, you will be expected to work collaboratively on complex cases with GPs, District Nurses and Therapists to work towards best outcomes for each service user.
Reducing the risk of hospital and care home admissions is a key goal the team works towards by having an asset based approach to each case.
Preventative measures are key to help reduce the risk of further harm or loss of independence for a service user.
About you
The successful candidate will have experience in a hospital or community based setting.
A degree within Social Work (Degree/DipSW/CQSW) with a minimum of two years experience working in an Adult's Social Work environment is essential.
A valid UK driving licence and access to a car is also preferred for this role but not essential.

About you
