About the team
The aim of this team is to provide quality support and protective intervention for vulnerable adults, families and carers.
The team manages a caseload of vulnerable adults, families and carers within statutory guidelines and local quality standards.
They also undertake direct work with individual service users, their families and/or carers as part of an agreed plan, and within this promoting health and wellbeing.
2 or 3 days a week will be spent in Bournemouth.

We are recruiting for a Social Worker to join a Leaving Care Team.
This role requires a Social Work Qualification and a minimum of 2 years of post-qualifying experience.
About the team
This team works closely with children in care and their carers to progress their care plans and to ensure that foster and residential homes are meeting the needs of children up to the time when they transition to leaving care.
As a ‘'corporate parent'' the role is to ensure that the children and young people is given every opportunity to achieve the best outcomes possible.
About you
The ideal candidate will have post-qualifying experience in child protection, children in need, safeguarding, looked after children or referral and assessment.
A degree in Social Work(Degree/DipSW/CQSW).
You will also need to be Social Work England Registered.

This team works vulnerable adults within the Adult Social Care setting.
This team aids the individuals by supporting them with their needs, the client may be 65+ or could have learning disabilities, mental health or physical disabilities.

About you
