We are looking for an Adult's Social Worker to join the Case Management Team on a locum basis based within London.

This post requires a Social Work Qualification of a minimum 2 year post experience.

About the team

This team works vulnerable adults within the Adult Social Care setting.

This team aids the individuals by supporting them with their needs, the client may be 65+ or could have learning disabilities, mental health or physical disabilities.

Responsibilities will include, face to face visits, carrying out assessments, creating care plans and working with other agencies for the benefit of the individuals.

About you

This post will require the candidate to have a social work qualification (Degree/DipSW/CQSW) with a minimum of 1 year post ASYE experience.

A candidate with experience working in complex care teams, LD, OP, dementia, Mental Health and Locality Team will be desired for this post.
